The username and password is for the server setup, see the jpg that is included by the author for more details... The only thing I changed was the MedicalCalendarFWG.exe file which was in the BIN folder if recall. G'luck.

Do I need to install both user and server sides to have this work. Secondly, I'd used the username and password on the read me and it does not work. Thirdly, thanks for the upload. Hope to hear from you.
